Ernie Clevenger- President & Ceo, CareHere

Since 1983, Mr. Clevenger has worked in the self-funded community in various capacities including: Chairman and President of the Self-Insurance Institute of America, Chairman of the Workforce for Electronic Data Interface (WEDI) advising United States Department of Health & Human Services, President of American Progressive Benefits and Executive Vice President of Alliance Underwriters and two of the nation's leading providers of stop loss insurance for self-insured employers. Mr. Clevenger and Ben Baker co-founded CareHere in January 2004.

Mr. Clevenger is a graduate of David Lipscomb University and received his MBA from the Owen Graduate School of Management of Vanderbilt University. He received his FLMI from the Life Management Institute.

Art, Assassination, and America: How Transparency Disrupts an Industry and Changes a Nation

Presented by: Martin A. Makary MD, MPH – Professor of Surgery, Johns Hopkins University School of Medicine

Dr. Martin “Marty” Makary is a physician, researcher and medical commentator. He is the Mark Ravitch Chair in Gastrointestinal Surgery at the Johns Hopkins Hospital, and teaches health policy at the Johns Hopkins University School of Public Health.

Dr. Makary speaks nationally on new technology in surgical care, quality and safety in medicine, and health policy. He is a regular medical guest on CNN and FOX News, and is the author of several books on public health and surgery, patient safety, quality of care, and health policy. His latest book, Unaccountable, came out in the fall of 2012.

Dr. Makary served in a leadership role at the United Nations World Health Organization (WHO) Safe Surgery Saves Lives Initiative. He is a leading advocate for transparency in medicine and common-sense solutions to healthcare’s problems, such as the Surgical Checklist, which he developed at Johns Hopkins.

His current research interests focus on the delivery of quality care, the science of medical errors and patient safety, international health, and health policy. He is internationally recognized for outcomes research and his work has been published in The New England Journal of Medicine and The New York Times.

Dr. Makary is also a champion for many innovative ideas in healthcare, including videotaping medical procedures so that patients can take home a copy for their records, lifting all restrictions on visiting hours in hospitals, as pioneered at Johns Hopkins, to promote family involvement, and allowing patients to see their doctor’s notes in real-time.

He completed his education at Bucknell University, Thomas Jefferson University, and Harvard University and his general surgery residency at Georgetown University. He then completed advanced training in pancreas surgery at Johns Hopkins before joining the faculty as a GI surgeon.

John Scott

John heads up the video team at Ocozzio. He is a 28 year old musician, writer and video marketer with outstanding communications and story-telling skills. He is a walking music and movie library with well-thought out ideas on what works and what is a waste of time. Ask about his dog, Nilsson, and his cat, Rufus Wainwright.  

Taylor Guido 

After a West Coast tour of duty with SAS and lululemon, Taylor joined Ocozzio's strategy team with a focus on developing “killer” messaging and winning marketing strategies. She’s a 27 year old kick boxer with a mean left. She also writes wickedly-smart marketing copy.

Zach Swenson

A world-class drummer by night, a top-drawer relationship manager by day. Zach's an older Millennial with two boys and a wife. A mortgage and a family give Millennials a totally different profile. Zach will provide perspective from the "other side" of 30.

Russell Krueger

We’re bringing him along solely because he’s paying for the trip. His day job is CEO of Ocozzio although many of us think he spends most of his time “entertaining" clients on the golf course. Just saying.

Presented by: Sal Nuzzo, Vice President of Policy at The James Madison Institute

Sal Nuzzo is the Vice President of Policy for The James Madison Institute, headquartered in Tallahassee, Florida. A native of high-tax/low-growth Connecticut, he left New England for Florida State University, a warmer climate and a healthier economy. Sal has spent his 20-year career engaged in the policy arena – in the Florida legislature as well as a policy consultant across the country.

Prior to joining The James Madison Institute, Sal served as a team member of the Florida Chamber of Commerce, where he directed the Chamber Foundation’s research efforts. When he's not busy trying to figure out the polar opposite personalities of his eight and seven-year-old daughters, he is working on the front lines of policy issues from health care to energy to criminal justice.

Sal earned a B.S. in economics with a concentration in Public Policy from FSU while serving with the Florida Legislature’s Office of Program Policy Analysis and Government Accountability (OPPAGA).

Lynn Kesterson-Townes is IBM’s Worldwide Commerce Marketing Leader focusing on the Insurance Industry.  Lynn has over twenty years of management and advisory expertise in marketing, market research, strategic planning, management consulting, business development, mergers and acquisitions, and project management. She has a proven track record as a line and project manager working on location in the Americas, Europe, Asia Pacific and Africa.  Lynn holds an M.B.A. from Harvard Business School; a Master of Science in Electrical Engineering from the University of Southern California; and a Bachelor of Science in Electrical Engineering from North Carolina State University.

Robert A. Mines, Ph.D., is the CEO of Mines and Associates (MINES). MINES is a national business psychology firm with two divisions: Organizational Psychology and Health Psychology. The firm offers organizational psychology services, training, employee assistance programs (EAP), managed mental health care, wellness programs, disease management, PPO services and a number of technology based services. In addition to his business and professional experience, Dr. Mines was an associate professor of Counseling Psychology at the University of Denver for 13 years, where he taught a variety of graduate level classes including group processes, counseling skills, assessment, practicum and internship supervision. 

Dr. Mines has published professional articles and presented at international and national meetings on topics such as organizational change and break through projects, succession planning, positive psychology and business performance, strategic alliances, marketing of professional firms, work stress, impairment at work, ethical issues, substance abuse and eating disorder treatment, health and wellness, intellectual development, emotional intelligence and law firms, emotional intelligence and leadership and numerous other topics. 

Dr. Mines is a licensed psychologist in the state of Colorado. He is a past president of the Colorado chapter of the Employee Assistance Professionals Association, was on the international board of directors, was the recipient of the 2004 International Employee Assistance Professional Association's Member of the Year Award and was the recipient of the 1992 Daetwiler Distinguished Service Award for his contributions to the EAP field. He is a member of the American Psychological Association and an individual member of the Employee Assistance Professionals Association.

Michael Meloch – President TPAC Underwriters

As the president of TPAC Underwriters, Michael Meloch has led the firm through a period of dramatic growth and consistent profitability. Over the last decade, TPAC has experienced a year-over-year annual growth rate of more than 12% and posted profits every year during that same period. Mike has grown TPAC’s annual premiums to more than $50 million since joining the firm in 1991 and launched a series of new product offerings including a capitated drug card, PPOx and Spaggregate®.


In 2005, Mike launched Risk Solutions Resources, Inc. (RSR) to serve as the special markets business manager for Pan-American Life Insurance. Offering accident and health products including stop loss, student accident, life, dental, and short-term medical, RSR has achieved 40% annualized growth since its inception. Prior to TPAC and RSR, Mike served in a variety of actuarial underwriting positions with MGUs and regional insurance companies.


A graduate of Bethel University, Mike and his wife Karin live in the Minneapolis area with their four children. Mike coaches a variety of youth sports, volunteers at his local church and serves on the boards of Bethlehem Baptist Church and University of Northwestern, St. Paul.

Park E. Eddy, MBA – President Active Captive

As the President for Active Captive Management (ACM), Park Eddy is responsible for the overall strategic development of the company and consults for its clients on strategic direction and operations. Park offers vast experience in all facets of the design, implementation and ongoing management of captives, as well as alternative risk transfer mechanisms and pools. He also collaborates with risk management departments or firms to design, implement, and manage ACM client’s insured and self-insured programs.

Park joined ACM as Executive Vice-President in 2003. Prior to that, he worked in the alternative risk market for Beecher Carlson and AON, and in the sales, management and marketing of pension investment programs for John Hancock (formally Manulife Financial) and The Guardian Insurance Companies. Park is well-regarded by his colleagues and industry professionals for his exceptional leadership skills and work ethic.


Park is actively engaged in the captive insurance industry and has been involved with the Alternative Risk Transfer Committee of the Self Insurance Institute of America (SIIA), the Rulemaking Advisory Committee for Captive Insurers of Oregon, the Enterprise Risk Captive Working Group, focused on government legislation regarding Captives, and the Captive Insurance Companies Association. He is a sought-after speaker, presenting regularly on complex industry matters and upcoming trends.


Park holds a Bachelor’s degree in Investment Finance and an MBA from the Marshall School of Business, both from the University of Southern California. He is also a licensed property and casualty and life insurance broker.

Kevin Larson – President, EBMS

Growing up within the walls of the family company gave Kevin the unique advantage of experiencing and learning every aspect of the business. He began his EBMS career in 1993 as an accounting assistant; and, in 1994, moved into the Marketing and Account Management Department, where he excelled in the positions of marketing assistant, marketing coordinator, and senior account manager.


Always a quick study, Kevin was promoted to director of operations in 2003; to director of account management in 2005; to vice president in 2007; then, in November 2010, Kevin’s years of experience, innovative implementation and dedication to EBMS earned him the title for which had worked for 17 years—president of EBMS.


EBMS President Kevin Larson manages the day-to-day operations of this fast-paced business in a rapidly-changing industry. He chairs EBMS’ five-member executive committee and acts as a conduit between EBMS’ operational management and ownership council. 


Kevin is actively involved with the Health Care Administrators Association (HCAA), serving as a member of their Past Presidents Council; the Society of Professional Benefit Administrators (SBPA); and, QUBIC, Inc.


Kevin has a Bachelor of Science degree in business management and is a licensed life and disability producer.

Healthcare Transition Committee - The ACA to Trump


Presented by: Tim Callender, HCAA Committee Chair and Board Member


Tim serves as the Vice President of Sales and Marketing for The Phia Group, LLC, headquartered out of Braintree, Massachusetts. Prior to his current role, Tim served as a Staff Attorney and Lead PACE Counsel for The Phia Group. 

Before joining The Phia Group in 2015, Tim spent years functioning as in-house legal counsel for a third party administrator. Tim is well-versed in complex appeals, direct provider negotiations, plan document interpretation, stop-loss conflict resolution, keeping abreast of regulatory demands, vendor contract disputes, and many other issues unique to the self-funded industry.

Tim has spoken on a variety of industry topics at respected venues such as the Society of Professional Benefit Administrators (“SPBA”) and the Health Care Administrator’s Association (“HCAA”). Tim currently sits on the Board of Directors for HCAA as well. Prior to his time as a TPA’s in-house counsel, Tim spent many years in private practice, successfully litigating many cases through full adjudication or to resolution through mediation or arbitration. 

Tim holds a leadership role with The Phia Group’s executive leadership team while continuing to assist on many general consulting and industry projects. Tim also employs his experience to focus on the development of new services and the enrichment of The Phia Group’s existing services.

Tim works out of The Phia Group’s newest office, in Boise, Idaho.

Hayes is the author of two New York Times best sellers: The Connection: How al Qaeda’s Collaboration with Saddam Hussein Has Endangered America and Cheney: The Untold Story of America’s Most Powerful and Controversial Vice President.


His work has been featured in The Wall Street Journal, The Washington Post, The New York Times, The Los Angeles Times, Reason, National Review and many other publications. Hayes has written extensively about national politics, international affairs and the country’s current political leadership. He was the first to report that Mitt Romney was preparing to select Paul Ryan as his running-mate and broke a series of stories related to the attacks on US facilities in Benghazi, Libya.


Syndicated columnist Charles Krauthammer said of Hayes' work on Benghazi: "if it doesn't win the National Magazine Award, the award ought to be repealed." The late Robert Novak called Hayes a must-read reporter on Washington and national affairs.

Jim Giordano - Managed Transitions

Jim Giordano is a healthcare executive with more than 25 years of experience in healthcare leadership. His background includes Partner and National Director of Value Based Care Services for an international healthcare strategy consultancy, as well as CEO, President, COO, and CIO C-suite roles in hospital systems and managed care organizations. He has experience leading organizations through significant market and leadership transitions. 
He has consulted on issues related to strategy, value-based care transformation, physician network development, value based IT infrastructure development, and product and partnership development related to fulfillment of accountable care and value-based payment strategies. Jim is a frequent speaker regarding the value based care market transition and is regularly referenced in the healthcare media. 


Bruce Butler - Santa Clara Valley Health Plan

Bruce Butler is a health plan general management executive with a career emphasis in provider-affiliated managed care.  He has deep functional expertise in underwriting, alternative risk transfer, and health benefits product design, applied to multiple market segments including all Commercial insured and self-funded lines of business, Medicare, Medicaid, and Workers Compensation.  His experience includes diverse enterprise models including publicly traded, private for-profit, non-profit, governmental, and joint ventures.  Currently, he serves as the Chief Executive Officer of Valley Health Plan, an HMO that is a department of the County of Santa Clara, California, with approximately 165,000 covered lives in government employer groups, Covered California (ACA Exchange), and Medicaid segments.  His previous roles include Chief Strategy Officer for medical centers for the University of California Office of the President, Chief Underwriter at Presbyterian Health Plan in New Mexico, as well as various underwriting, product development, provider network development and related roles with leading national payor organizations, excess risk markets, and entrepreneurial managed care ventures.  He has a BA from Stanford University and an MBA from the University of Chicago.
